About Us

Coordinated Family Care (CFC) is a private non-profit Care Management Organization funded by the NJ Department of Children and Families. We provide guidance, options, and assistance to Middlesex County families that care for youth who present with very high levels of behavioral and/or emotional needs. Care Management Organizations are an important part of a state-wide effort to improve New Jersey's coordination of care for youth and their families.

CFC's approach to helping families is grounded in a philosophy that begins with three key elements:

  • Choice
    To provide the right services to families at the right time, CFC has developed relationships with many types of providers throughout the county.
  • Voice
    CFC works with the family and whoever the family wants involved (aunts/uncles, neighbors, friends, clergy, etc.) to develop a plan of care.
  • Resources
    We help families access creative, non-traditional services. CFC will also help families apply for and obtain state benefits that will assist with longer term care that may be needed.
