Community Hope Veterans Suicide Prevention Program

SERVICES OFFERED:

  • Outreach to identify those at risk of suicide.
  • Case management services and VA benefits assistance for eligible individuals and their families.
  • Education on suicide risk and prevention to families and communities.
  • Provision of clinical services for emergency treatment.
  • Assistance with obtaining and coordinating benefits provided by the government, or an eligible entity.
  • Assistance with emergent needs for health care services, daily living services, personal financial planning & counseling, transportation services, fiduciary and representative payee services, legal services to assist with issues that may contribute to the risk of suicide, and child care.
  • Family members complete an intake form and mental health screenings; however, family members do not need to also be at risk for suicide.

The service member or veteran’s eligibility allows a family member over 18 to receive services that are in support of and related to the eligible individual’s risk mitigation.
