ModivCare Medical Transport

Welcome to the Modivcare Member website for New Jersey. Currently, this site hosts information for members about how to get a ride if they have no other way to get a ride.

Please call to schedule a reservation. All calls must be placed at least 2 business days before the transportation will be needed.

Please have this information available when you call to reserve your ride:

  • Your first and last name, date of birth and phone number
  • Health care plan and member ID # (you can find this on your health plan card)
  • Date and time of your clinical visit
  • Name, phone number and address of the clinical provider (Where are you are going for your visit?)
  • General reason for the appointment
  • Level of service. (Are you able to walk or do you use a wheelchair?)
  • Pickup time and location
  • Know if you need a one-way or round trip
  • Any special instructions, if any (including information about any medical equipment you will bring, e.g. wheelchair)

Available transportation will depend on your location and condition and may include:

  • Car, vans, taxi or rideshare service
  • Ambulance (ALS / BLS), mass transit, specialty care transport
  • Wheelchair lift-equipped vehicle

There is no additional cost for this service beyond the current health plan. However, there may be a limit to the number of trips a member can take annually. A trip is considered one way; a round trip would be two trips.
Please contact Customer Service at the number on the back of the member ID card to determine how many trips are included in the plan.
This is a curb-to-curb service. Drivers are not allowed to enter a member’s home or medical facility. Drivers are required to wait at most 15 minutes past the scheduled pick up time.
