The Sustainable Health & Wellness Village

The SHWV has been providing access to primary care and specialists, healthy nutritious food, family care initiatives and more since 2014. It expanded into a statewide multi-day community-based health and wellness festival that has been operating in underserved communities since 2021. We have impacted over 50 thousand residents by hosting a culturally relevant, festival-like atmosphere designed to promote meaningful engagements between communities of color and local and regional provider.

Free Fresh Produce Market For Seniors and Families

Servicing Newark & greater Newark communities, this ongoing initiative is a collaboration between CareSparc Community Connections and Table to Table, which directly addresses health equity barriers in underserved communities through innovative partnerships and community engagement approaches. These gateways facilitate linkages to social and economic opportunities, as well as community resources that impact health in a coordinated manner for participants at pre-selected subsidized senior and family buildings.

Artists & Creatives

Through a series of comprehensive healthcare workshops in the communities of Newark, New Brunswick, and Jersey City, this growing initiative provides artists and creatives with educational resources to help them adequately prepare for their future. CareSparc Community Connections acts as the convener of community resources to help Artists and Creatives gain healthcare and financial literacy. During these in person events, artists and creatives have access to start the enrollment process with a health insurance provider, receive a one-on-one consultation, and obtain information and tools that assist with navigating the complex healthcare system.

Grandparents Raising Initiative

Servicing the Newark community, this continuing initiative addresses food insecurities and provides access to healthy food resources for older Americans ages 60 and older. The program is geared to assist grandparents in navigating systems to help them to be strong advocates for themselves and for the children in their care by leveraging the SHWV model.
