Nurse-Family Partnership

Nurse-Family Partnership helps transform the lives of vulnerable first-time moms and their babies. Through ongoing home visits from registered nurses, low-income, first-time moms receive the care and support they need to have a healthy pregnancy, provide responsible and competent care for their children, and become more economically self-sufficient. From pregnancy until the child turns two years old, Nurse-Family Partnership Nurse Home Visitors form a much-needed, trusting relationship with the first-time moms, instilling confidence and empowering them to achieve a better life for their children – and themselves.

 This evidence-based model of partnering nurses and first-time moms has more than 35 years of research from randomized, controlled trials that prove it works. And these moms and their babies are not the only ones who benefit. Communities and society as a whole have grown stronger thanks to Nurse-Family Partnership's commitment to achieving the following goals:

  • Improve pregnancy outcomes by helping women engage in good preventive health practices, including getting prenatal care from their healthcare providers, improving their diet, and reducing their use of cigarettes, alcohol, and illegal substances
  • Improve child health and development by helping parents provide responsible and competent care
  • Improve the economic self-sufficiency of the family by helping parents develop a vision for their own future, plan future pregnancies, continue their education, and find work

The Nurse-Family Partnership National Service Office provides training and support services to ensure that the model is precisely replicated in communities, leading to improved outcomes for both mothers and children. Sites adhere to key elements of the Nurse-Family Partnership model, which include:

  • Enrolling first-time, low-income moms early in their pregnancies
  • Specially trained public health nurses delivering home visits over two-and-a-half years
  • Establishing support for the program within an implementing organization

Eligibility You must be a first-time mom within the first 28 weeks of pregnancy to be eligible for the program.
