Integrated Case Management Services (ICMS)

Population Served: Severely and persistently mentally ill adults (ages 18 and over) residing in Middlesex County who have been discharged from a psychiatric hospital or are at risk of hospitalization.
Referral Source: State or County Psychiatric Hospitals, Short Term Care Facilities (STCF), and Local Mental Health Service providers
Funding Source: New Jersey Division of Mental Health and Addiction Services (DMHAS)
Number Served: Case load capacity is 420 active cases.
Service Provided: Outreach, linkage to mental health and other supportive services needed to prevent re-hospitalization and optimize success in community settings.
Length of Service: Approximately 12 months
 

Homeless Case Management (PATH)

Population Served: Severely and persistently mentally ill adults (ages 18 and over) who are homeless or at risk of homelessness in Middlesex County.
Referral Source: Referrals are accepted from outpatient or inpatient providers, social service agencies, homeless shelters, soup kitchens; particular emphasis on those without mental health services. Individuals seeking service can refer themselves as well. Funding Source: New Jersey Division of Mental Health and Addiction Services (DMHAS)
Number Served: A total of 95 individuals to be served per year.
Service Provided: Outreach, linkage to mental health and other supportive services that are geared towards the maintenance of housing and acquisition of independent living skills while optimizing success in community settings.
Length of Service: Approximately 6 months after housing and mental health linkage are successful.
 

Intensive Family Support Services (IFSS)

Population Served: Family members of mentally ill adults (mentally ill member must be 18 or over) residing in Middlesex County.
Referral Source: Any source, families or individuals seeking service can refer themselves; referrals are also accepted from outpatient or inpatient providers and other social service agencies.
Funding Source: New Jersey Division of Mental Health and Addiction Services (DMHAS)
Number Served: 150 new families per year, with a total of 250 families receiving various services per year.
Service Provided: Family support, advocacy and education, multi-family groups, educational conferences, referral and linkage.
Length of Service: Depends on individual case needs and preferences.
 

Specialized Case Management and Outreach Services (SCMOS)

Population Served: Young adults, adults and the elderly who are unengaged and needing mental health services in Middlesex County.
Referral Source: Individuals or families seeking service for an eligible individual can make referrals. Referrals are also accepted from outpatient or inpatient mental health providers, social service agencies and Aging-In Agencies.
Funding Source: The County of Middlesex
Service Provided: Outreach services are geared towards successful linkage to mental health and other supportive social services.
 

Justice Assistance Grant Program (JAG)

Population Served: Adults (ages 18 and over) diagnosed with a serious and persistent mental illness in Middlesex County Jail being released into the community.
Referral Source: Middlesex County Jail
Service Provided: Pre-release screening and aftercare planning, referral and linkage to mental health or co-occurring substance abuse treatment programs, coordinating release planning with Prosecutor, Public Defender, criminal justice system personnel and family; case management and community-based support after release from jail, assistance with housing placement and employment counseling with 24/7 crisis intervention.
Funding Source: New Jersey Division of Mental Health and Addiction Services (DMHAS)
Number Served: Case load capacity is 40 individuals.
Length of Service: Up to 12 months
 

Residential Services

Population Served: Severely and persistently mentally ill adults (ages 18 and over) who are:

  • Currently on CEPP status at a state hospital
  • Homeless or at risk of homelessness


Referral Source: State Hospital, community based providers of homeless services
Service Provided: Case management services to include voucher management, skills building, advocacy, medical and behavioral health supports and linkage to community based providers.
Funding Source: New Jersey Division of Mental Health and Addiction Services (DMHAS)
Number Served: Dependent on specific grant
Length of Service: Dependent on individual consumer needs
